Emerging Strategies for Addressing Older Adult Homelessness
Description

The Michigan Coalition Against Homelessness and Community Housing Network are leading a pilot program in Oakland County focused on older adults facing homelessness and health inequities. The initiative is creating stronger collaborations between housing and health systems through enhanced coordination and data sharing, improving outcomes for unstably housed older adults who frequently access healthcare services. Best practices for wraparound services, based on person-centered advocacy, are being developed and documented. The initiative seeks to allow organizations throughout Michigan to connect with this work and identify systems changes that could improve outcomes for older adults in their communities. The pilot is funded by the Michigan Health Endowment Fund.
