The circumstances of peoples' lives, also known as the social determinants of health (SDOH), are known to greatly influence health outcomes. The 2022-2024 MDHHS Social Determinants of Health Strategy, Michigan's Roadmap to Healthy Communities, created a plan to align efforts at the state and local levels for greater SDOH improvements in communities. The Roadmap recognizes that Housing instability and homelessness are inextricably linked with Food Security and Health Equity, and many homes have hazards (such as lead and carbon monoxide) that can increase risk for injury, illness, and chronic disease. Housing instability can also perpetuate a cycle of reinforcing crises: an individual's health problems can lead to a homelessness and homelessness can lead to further health problems. Equitable access to safe affordable housing and nearby healthy, low-cost food are key to improving health and social outcomes. Access and navigation of healthcare systems and social supports, including housing assistance, are also crucial for health equity and can be fostered through the expansion and alignment of Community Health Workers across the state. Finally, the voice of community members experiencing illness and housing challenges, as well as the local organizations that help close these gaps, must be included in policy change and statewide program development, if they are to accelerate our collective efforts to prevent chronic disease.
