Two MDHHS Policy and Planning officials will present on 1) Core principles behind the MDDHS Social Determinants of Health Strategy and 2) Goals for Phase 1 and the plans for Phase 2 of the Strategy. How does data gathering and health IT figure in the state’s plan? How can housing stability alleviate chronic disease burden in Michigan? The 2nd part of the Session will include the opportunity to give MDHHS input on regulatory barriers and policy change opportunities that can help the State of Michigan leverage SDOH programs and new funding to support summit attendees in their day-to-day work.
