Jennifer Chaffer serves as the Director of Community Health & Well-Being at Trinity Health Grand Rapids hospital. She is a licensed social worker, and has been with the organization for 24 years in both clinical and leadership roles.

As the leader of Community Health & Well-Being, she coordinates the hospital’s engagement in—and response to—the Kent County Community Health Needs Assessment (CHNA), ensuring the priorities of the community are at the center of the hospital’s community outreach and engagement efforts. She is excited by the opportunity to collaborate with community partners to address the needs of those living in Grand Rapids and Kent County, and loves finding ways to make a positive difference, even in the face of big challenges and limited resources.

Jennifer holds a bachelor’s degree in sociology and English, and a Master's degree in social work, both from the University of Michigan.
